Hello! Thank you for your interest to schedule a Sozo session.

Our Mission

Session Donation

For each session, we request a $30 donation. You can pay either in-person with cash/check or online with a credit card. However, other arrangements may be discussed with the Sozo leader. Please note that donations are not tax-deductible.


Session Changes

After you submit this form, we will contact you to schedule a session. We request a 48 hour notice from you if you need to change or cancel your confirmed scheduled time. If cancellation occurs within the 48 hours, the $30 donation would still be required.


Purpose of this Pre-Ministry Form

This form helps the Sozo team prepare for your session. All information on this form is kept confidential between team members. The only exception is if we have to legally share information.

I acknowledge that team members from the Sozo ministry of Encounter Jesus have voluntarily agreed to pray for me. I understand sessions are not a professional counseling meeting and that none of the team members are licensed counselors. I understand that these team members are, to the best of their ability, doing what they can to help me achieve more freedom in my life.

I further state that I have voluntarily sought assistance of my own initiative and that I am under no obligation to accept or reject any of the advice or help that I might receive from the team members of this ministry.

All personal information gathered in a ministry session remains confidential. Prayer ministers can consult with other team members or Encounter Jesus elders if needed.

I understand that Encounter Jesus reports to the proper authorities if needed to comply with legal regulations. This includes but is not limited to reporting of child/elder abuse, criminal activities, and threats of harm.

I agree to hold Encounter Jesus and its team members free from any and all liability, loss or damage of any kind that may arise as a result of assistance, which I have received, or from my involvement with Encounter Jesus.
